Skilled Nursing Facility (SNF) and Long Term Care (LTC)

 

Lefa L. Seran Skilled Nursing Facility has 24 beds and offers comprehensive health services for short term, long term, and special care needs.  It is our mission to make a positive difference in the life of each individual that has been entrusted to us for care.  We are a "home" in the old-fashioned community sense, and the cultural values of our residents are a focus of daily life and activities. 

 

Mt Grant General Hospital and Lefa L. Seran SNF are located in the same one story building.  The medical clinic with suites for 4 providers is situated just feet away. The local dentist and the public library are just across the parking lot as is the building that houses medical specialists that visit our rural community on a regular basis.

 

We have low staff turn over and good staff to resident ration.  Meals and snacks are customized to suit each resident’s preference.  The open unit provides security to residents that may wander through an alarm system that alerts staff if a resident in danger of wandering leaves the unit.  Alarms are also used instead of restraints to reduce the risk of falling.  The physical environment is exceptionally clean and fresh.

 

We are committed to providing the best quality of care possible thru personalized nursing and medical care, as well as an innovative rehabilitation program.  Our Director of Nursing is a Registered Nurse, and leads a team of professional licensed nurses.  Our physical therapist, dietitian, and social worker work closely with our nursing staff to ensure an interdisciplinary approach to our residents.  The restorative nursing care not only improves our residents' functional abilities and independence, but also affords many of them the opportunity to return to the community.
